To All Licensed Midwives:  Informed Consent Required:


The following contains the requirements from Rule 325.01 to be included in each midwife’s personal informed consent.  This is to be given to the client, read, signed and returned to the midwife before starting care.  Please make sure these items a.-g. are included in your informed consent forms.  If you have a question or would like help, I would be happy to assist you, please contact me:  Annette Lewis, LM, Midwifery Review Board:

a.  The licensed midwife’s training and experience.

b.  Instructions for obtaining the Boards rules (www.ibol.idaho.gov. scroll to Midwives, click, then go to State Licensure Rules and click.)

c.  Instructions for obtaining a copy of the Essential Documents of the NACPM (go to www.NACPM.org, click on A-Z Index, click on ESSENTIAL DOCUMENTS (Philosophy , Scope of Practice, Standards of Practice), Analysis of the 2001 Job Analysis Survey, published by NARM (go to www.narm.org, click on Technical Documents, go to NARM Job Analysis: 2001 Job Analysis, pdf file).

d.  Instructions for filing  complaints with  the Board (go to www.ibol.idaho.gov, go to Midwives, scroll all the way down on the left to “Filing a Complaint”).

e.  Notice that the licensed midwife does or does not have professional liability insurance coverage.

f.  A written protocol for emergencies, including hospital transport that is specific  to each individual client.

g.  A description of the procedures, benefits and risks of out-of-hospital birth, primarily those conditions that may arise during delivery.

Rule 325.02 Record of Informed Consent: All licensed midwives must maintain a record of all signed informed consent forms for each client for a minimum of 9 years after the last day of care for such client.
